I did it once to an epically rude waitress at a Denny's when I was about 20.
A group of four of us totally walked out. What can I say? We were young, on a lethal mix of Black Velvet and cocaine, and grouchy.

I did it in HIGH SCHOOL, never as an adult. It was on a UIL trip and I wanted to keep my meal allowance.

In college we were really lit one night and this was in downtown Chicago and we were eating at this Polynesian place called Jimmy Wong's in the Loop. Irt was really late, probably around the time the kitchen was closing, we ordered all this stuff, ate it, and when our waiter left we ran like hell. I felt like I was going to puke because I was so scared. To this day I feel so guilty!!! Jimmy Wong's rocked tho, great food...

I've never dined and dashed. I forgot about a bar tab once, but it wasn't intentional.

When I was in high school, a group of us skipped school and went out for lunch. Our waitress was this horrible old woman who just gave us all the icy stare the whole time we were eating. For her tip, we spelled out "Bitch" in pennies.
